Will Levis' surprise addition to injury report raises red flag on Titans injury management

When Brian Callahan told media on Friday that a couple players would be listed as QUESTIONABLE on the final injury report, nobody was expecting Will Levis to be one of them.
But there he is. After a limited-full-limited week of practice participation, the Tennessee Titans starting QB is headed into the Week 7 matchup in Buffalo as questionable.
His listed injury is a right shoulder, the same throwing shoulder injury he's been nursing since he fell hard in Miami nearly 3 weeks ago. after the Titans Week 5 bye, he was a full participant in all three days of practice before playing in Week 6 against the Colts. He had no designation going into that game.
So what happened? Did Levis reaggravate his shoulder injury this week? Has the Titans coaching and medical staff determined they were wrong to let him come back so early? Are they fearful for the rest of the season because it's not seeming to heal?
Whatever the story here, it makes the Titans decision to roll Levis out against the Colts look extremely questionable. Some were already questioning the decision not to let Levis rest an extra week and instead have him play less than 14 days after the injury occurred. And that was before he played in a loss where at least a pair of passes looked to potentially be impacted by a passer who wasn't 100%. Levis even pointed to his unhealthy shoulder after the game as a reason he threw an interception.
This seems like a bad look for the Titans no matter how you slice it. If he aggravated his shoulder in some way between last week and today, and that's the reason why he suddenly has a designation he didn't previously have, fair enough. But that sounds like a situation where you should have considered resting him in Week 6 so that he wouldn't be at risk of aggravating the issue in the first place!
And an even worse possibility is that they're simply backtracking on what they think he can do. If the injury hasn't changed at all, but last week's performance makes them now believe he may not be healthy enough to go, then he wasn't healthy enough to go last week! And you played him anyways.
It's a confusing, frustrating situation all around. There doesn't appear to be a satisfactory explanation for this, unless the injury is somehow brand new. It looks like the Titans mismanaged, at least to some extent, the return of their starting QB. And now his health is trending in the wrong direction.
